Members Internal Event Reservation-Request

This form is for members ONLY hosting internal church ministry events. If you are a member and would like to host an event at Revive Church for your personal ministry/business, please complete the external request form.

To secure an event on the calendar or if your event is already on the calendar, this form must be completed.

The following steps should be followed to ensure
proper coordination and maximum success of the proposed event.

  1. Please complete & submit this form at least 60 days prior to the event to ensure all necessary modes of coordination can be achieved.
  2. Please do not advertise the event until you have received confirmation.
  3. If there's a speaker, please submit with this form. 
  4. Requests are reviewed & approved by the Leadership team.
Please note that submitting this form doesn’t mean your request is approved. You'll receive an approval notice from either the Leadership Team or our Revive Reservations Team.

If the speaker(s) isn't a Revive Member, please enter details below, and this will be reviewed by the Pastors & Leadership Team.
